Transition Program Coordinator Moving People from Nursing Homes to the Community. Half time position Summary: Assist Medi-Cal eligible skilled nursing facility residents who wish to transition to community based living. Based in Santa Barbara headquarters office, serving the Tri-County area 20 hrs/wk $1,274-$1,386-/mo to start, DOE. Paid holidays, sick/vacation If successful, position could increase to 35 hours/wk within 6 mo which would be eligible for health insurance benefits. Deadline to apply: 2/24/12. Essential Duties:
Make contacts/develop working-relationships with nursing facility social service workers throughout the tri-county service area and periodically visit each location
Respond to and screen transition candidates
Present the transition program to resident council meetings
Provide initial Program information, orientation and interviews required by the program
Coordinate with ILRC staff to assist transition participants with IL services
Perform all related record keeping and data collection activities
Monitor transitions, maintain contact, provide peer support to participants and handle reporting
Assist participants setting and attaining goals
Required Qualifications:
Have and use excellent communication and organizational skills
2 or more years experience in Independent Living or related field
Ability to meet deadlines, balance multiple tasks
Ability to implement Independent Living model of service provision
Experience working with persons with disabilities
Ethical work practices: honest, dependable, team-oriented work style.
Ability to arrange for/provide own transportation throughout service area.
